    syncretro: record what the milestone docs' deferred items became

    M2_INPUT.md, M3_MULTICORE.md and M4_AUDIO.md are the design record the
    door's code cites by section for its defaults, its measurements and its rejected alternatives, so their "Deferred, with reasons" tails get read
    as current. Several of those items have since shipped while still
    reading as open work:

    - the volume hotkey (M4) -- '+'/'-' walking a 0..100 percent
    - the CP437 text tier (M2) -- charset taken from the client, F4 cycles
    - core options and save states (M3) -- the option store, and suspend as
    an automatic per-user snapshot
    - the NES install (M3), which had no cartridges and no xtrn.ini entry
    - the Intellivision keypad's end-to-end semantics (M2), blocked at the
    time on a BIOS the install now carries

    Each item keeps its original reasoning and gains what actually happened, including where that differs from the plan: save-state hotkeys and a save
    UI are still open, and a descriptor-derived per-game legend was answered
    from the other end by games.ini, for the console that sends no
    descriptors at all. Two section cross-references corrected in passing.
